Providing this special type of care, comfort, and supportive services requires the coordinated efforts of individuals with different areas of expertise:

medical professionals and caregivers specializing in pain management; aides to assist in activities of daily living; dietary personnel; specially trained and experienced social workers; chaplain or clergy members; and of course, the active involvement of family members themselves.

We can provide services in your home or in the care facility that your loved one is admitted to. There are several levels of care that we can provide, including but not limited to:

  • Routine Home Care – is care provided to a patient in the patient’s own environment including home, skilled, or assisted living.
  • Continuous Care – is provided in the patient’s environment on a short-term basis when medical symptoms are not controlled at the routine home care level. The objective of continuous care is to maintain the patient in their own environment. Nursing care is primarily provided from 8 to 24 hours in a day to achieve palliation or management of the acute medical crisis.
  • Respite Care – Temporary or periodic care provided in a nursing home, assisted living facility, or other type of long-term care program so that a family member or friend who is the patient’s caregiver can rest or take some time off.
  • General Inpatient Care – Short-term inpatient care is available for pain control and symptom management that cannot be controlled in home environment. Once the symptoms are resolved and the plan of care is unchanged within 24 hours, the patient is discharged back to the home or facility.

As part of our commitment to servicing the community at large, Aserenity Hospice Care offers several community based services, including:

  • Community Education – A variety of educational programs geared to help the community at large understand and appreciate the medical, social, economic and spiritual issues that hospice patients and their families must deal with. To arrange a program for your organization, please contact us.
  • In-servicesAserenity employees and caregivers are given specialized hospice trainings and updates relating to hospice standards of care and treatment. They are acquainted with the most current approach in social and hospice care methods, so that they can serve patients and their families optimally.
  • Bereavement Services – In times of grief, Aserenity Hospice’s professionally trained grief counselors are available for comforting family and community members. They specialize in providing condolence and consolation to family members and close friends of the departed.
